OUTLINE OF BENEFIT PLANS

All Plans used in KHIA are preferred provider plans. KHIA utilizes two preferred provider networks. Upon acceptance of your application, you are assigned either First Health or Preferred Health Care depending upon your demographic location. These plans give you the option of choosing any provider but pay a higher percentage of allowed charges if you choose a provider who is part of your network. Offered plans include deductibles that range from $1,500 up to $10,000. These plans are not available to Medicare eligible enrollees.
